Current Opportunity: Housing Retention Case Manager (Full-Time)
Interfaith Hospitality Network is seeking a Housing Retention Case Manager to assist client families who have left the shelter program but are still in need of assistance to retain their housing and to become stable. An LSW is preferred; a bachelor's degree is preferred; experience related to this position is required.
Duties:
To provide case management and crisis intervention services to homeless families currently participating in or recently discharged from the Interfaith Hospitality Network of Greater Cincinnati in order to assist them to obtain and retain housing. This is a highly responsible professional position providing assessment, management and advocacy services to homeless families with children. The Going Home, Staying Home Case Manager will work with other professionals from numerous community organizations, with volunteer Family Mentors, with volunteers from the IHN member congregations, as well as IHN staff. Duties are performed with a high degree of independence under the supervision of the Executive Director. The expected outcomes of this position are that families will obtain and retain housing upon leaving Interfaith Hospitality Network of Greater Cincinnati (IHNGC).
Requirements:
- Licensed Social Worker (LSW) (strongly preferred)
- Five years or more experience in comparable position
- BSW (experience and other education such as CEUs may be substituted for BSW)
- Background investigation.
Responsibilities:
Team : Participate in the full functioning of IHNGC by affirming and assisting the contributions of others
Case Management:
- Meet with Shelter Case Manager and Executive Director to review new guests
- Develop individual case plans for assigned families admitted to and/or leaving IHNGC (includes the goals, objectives, steps and timetable the family will follow)
- Review the case plans with assigned families weekly
- Advocate on behalf of the guests as needed to help them access essential social services and other programs
- Discuss any concerns or problems with the Executive Director.
- Long term follow-up case management with assigned families
- Teach classes as necessary for guest families
- Meet with assigned former guests on a weekly basis for at least 3 months, then as determined.
- Maintain weekly contact with guests by phone after weekly meetings changed to different schedule.
- Match families with Family Mentors before families leave IHNGC
- Assist in implementing Family Mentor program
- Train and supervise Family Mentors
- Serve as liaison with Family Mentors.
- Maintain records, including HMIS/Vesta.
After Care:
- Manage a support and information system for guests who have exited IHN
- Track the success of families who have exited the program with regard to permanent housing and continued employment, through weekly contacts the first 6 months - then monthly for an additional 6 months
- Maintain tracking information and other records on secure management information system
- Monitor and evaluate program as necessary.
External:
- Act as a resource for other agencies seeking referral information on current and former guests
- Participate as a member for IHN with assigned organizations by attending meetings and providing data
Day Center Operations:
- Sharing operational tasks as needed
- Coordinate guest's classes on money management/renter's rights/child care
- Maintain records on guest's progress. Provide reports to the Executive Director and/or the Board of Trustees, as required.
- Be on call for emergencies when the Shelter Case Manager is unavailable
Hours: 40 hours per week; flexible
Accountability:
- Accountable to the Executive Director of Interfaith Hospitality Network of Greater Cincinnati
- All employees are evaluated on a yearly basis
